Expression of Interest โ Bellwether Surgery Accessibility & Financial Protection Study (TORG-ETH)

Project Summary: Bellwether surgical conditions (emergency laparotomy, emergency caesarean section, and open fracture fixation) are vital to saving lives in low- and middle-income countries. However, in many African communities, people face long travel distances and severe financial hardship to access these services.
This multi-centre, prospective study will examine geographic accessibility and financial risk protection for bellwether surgery in resource-limited hospitals across Africa.

This project builds on a successful ๐ฌ๐ข๐ง๐ ๐ฅ๐-๐๐๐ง๐ญ๐๐ซ ๐ฉ๐ข๐ฅ๐จ๐ญ ๐ฌ๐ญ๐ฎ๐๐ฒ at Queen Square, London, where ๐ฏ๐ข๐๐๐จ-๐๐๐ฌ๐๐ ๐๐จ๐๐๐ก๐ข๐ง๐ ๐๐ง๐ก๐๐ง๐๐๐ ๐๐ฒ ๐๐ was shown to improve both surgical performance and patient outcomes in ๐๐ง๐๐จ๐ฌ๐๐จ๐ฉ๐ข๐ ๐ฉ๐ข๐ญ๐ฎ๐ข๐ญ๐๐ซ๐ฒ ๐ฌ๐ฎ๐ซ๐ ๐๐ซ๐ฒ. With early feasibility demonstrated, we are now moving toward ๐ ๐ฆ๐ฎ๐ฅ๐ญ๐ข๐๐๐ง๐ญ๐๐ซ ๐ฌ๐๐๐ฅ๐-๐ฎ๐ฉ, with a focus on ๐๐จ๐ฐ ๐๐ง๐ ๐๐ข๐๐๐ฅ๐-๐๐ง๐๐จ๐ฆ๐ ๐๐จ๐ฎ๐ง๐ญ๐ซ๐ข๐๐ฌ (๐๐๐๐๐ฌ).
We are seeking institutions that meet the following ๐ฆ๐ข๐ง๐ข๐ฆ๐ฎ๐ฆ ๐๐ซ๐ข๐ญ๐๐ซ๐ข๐:
- Perform at least one transsphenoidal pituitary surgery per month.
- Have at least two consultant-level neurosurgeons involved in pituitary surgery.
- Are in a low- or middle-income country (as per World Bank classification).
- Do not currently have an existing video-based surgical coaching program.

The Operating Room Global (TORG), as a proud member of the G4 Alliance, is thrilled to announce that 19 hospitals from across our community have stepped forward to participate in the WHO Clinical Registry: Operative Encounter Module Pilot!
This groundbreaking initiative, developed under the guidance of the WHO Clinical Services and Systems Unit with technical inputs from the G4 Alliance, aims to establish a global dataset for operative encounters, driving quality improvement, upskilling, advocacy, and surgical system planning globally.
These institutions will be pioneers in shaping the future of surgical, obstetric, trauma, and anesthesia (SOTA) care through data-driven decision-making. They will also engage in the Community of Practice (Project ECHO model) to share experiences and learn collectively.
